Nutcracker Absence Request Form

*This form must be submitted at least ONE FULL week prior to the requested absence*

You must submit this request online ONLY (phone calls, verbal statement or e-mails will NOT be accepted).

Absences from rehearsals are only excusable under special circumstances. On occasion, even when all the requirements for an excused absence have been met, the Artistic Director and/or School Director may still deny the request if he/she believes this absence would weaken the efficient progress of rehearsals or negatively affect the success of a performance.

Remember – NO unexcused absences will be allowed. Unexcused absences will result in removal from choreography and/or a performance.
